ENROLLMENT INFORMATION
Enrollment is limited and taken on a first-come, first-served basis, subject to availability. However, priority enrollment is given to currently enrolled families and staff members. If you are interested in enrolling, please call us at 845-2367 to check on available openings and to arrange a time to come and tour the center. Feel free to bring your child so that he or she can see the center. Please allow at least 30 minutes for your visit.
If there are openings, you will receive an Application for Enrollment. After submitting your application and registration fee, parents will receive a confirmation of their registration, final enrollment forms (medical forms, etc.) and a copy of the Parent Handbook. It is expected that parents will be familiar with and agree to all policies.
If there are no openings currently available, your name (child’s name and birthdate) can be added to the waiting list. There is no charge to be put on the waiting list. For those on the waiting list, parents will receive Parent Handbooks and enrollment forms when your child is placed in the program.
Once Upon a Time is not able to hold spots for any reason at any time. Please discuss any such situations with the Center Director.
Prior to your child’s first day at the center, please feel free to schedule times when you can bring your child in for a visit. It is a much smoother transition for children, and for all, if the children have been to the center prior to their first attendance day.
The first week’s tuition is due the Friday before the first day of attendance. After that, tuition is due each Monday morning for that week of care. You may place all payments in the mailbox located in the reception area next to the front door.
SCHEDULE:
Once Upon a Time develops class schedules in February of each year. Current families fill out registration forms at that time for summer and fall/spring programming. Once current family enrollment is complete, open spots will be filled by the waiting list and then the general public.
Only full time care is offered for children in the Infant and Toddler classrooms (approximately 6 weeks through 24 months). Full time children may not exceed 10 hours of care per day. Part time care must be either full days, mornings (between 6:30 – 12:00 a.m.) or afternoons (between 12:30 – 6:00 p.m.). Part time families may add a day but may not change or switch days during the week.
Changes in enrollment must be communicated to the Center Director as soon as possible. Please do so by either writing a note and leaving it with the Center Director or by writing a note in the Parent Communication Log located at the reception desk.

- Child Health Report (Dated no more than 6 mo. prior to attendance and no later than 3 months after the first day of attendance. Must also be signed by a doctor.)
- Daycare Immunization Record (within 30 school days after enrollment)
- Child Health History and Emergency Care Plan Form
- Authorization to Administer Medication
- Child Intake Form (children under 2 years of age)
- Ointment/Powder Permission Form (children in diapers)
- Sunscreen and Insect Repellent Permission Form (all children)
- Walking Field Trip Permission Form
- Use of Name and Media Release Form

** NOTE: Parents will be asked to update these forms as required.
Updating Forms: It is extremely important to keep the Center Director current on all phone number and address changes. Please put these changes in writing in the Parent Communication Log located at the reception desk. Child Intake forms for children under 2 years of age must be updated every three months. Your child’s teacher will provide you with a copy of the form to be updated. Please complete and update the form promptly and return it within 10 days to your child’s teacher.
The programs at Once Upon a Time are inclusive of all children, including those with identified disabilities and special learning and developmental needs. The Center will make reasonable accommodations to meet the needs of children with disabilities as described in the Americans with Disabilities Act. All staff assigned to special needs children will be aware of their special needs (physical, health, mental) and any intervention plans. Staff may make appropriate professional referrals when necessary. Family members are involved in the decision making process and in making and using Individualized Education Plans (IEP’s).
